1) Data definition code must be same as the concurrent program short name. Here the data definition code is case sensitive so we have to care about that.
2) Sometimes the concurrent program is not taking the rtf template then we have to follow following steps:
a) Go to navigation : system administration ->concurrent ->program

b) Query for your program
c) Click on update
d) Click on onsite setting
e) Select your template and apply
3) The last mistake which we usually doing is to change the output format in concurrent program. By default it will be in text format change to xml format.
